A customer sees Approved but cannot enter the service. A fresh agent can change code quickly. The operator first needs to know whether the fault sits in policy, effect delivery or the read model.
One screenshot is compatible with a denied policy decision, a lost entitlement job or a stale status projection. Patching the display may make the demo green while leaving the customer blocked.
Give the request a stable identity. Preserve the versioned policy decision, the entitlement application receipt and a read from the service that controls access. Find the first divergence before generating a repair.
Require a regression case that reproduces the divergence and turns red without the fix. Do this because a code patch is only a hypothesis until an independent state readback proves the customer's path recovered.
