Yesterday's cohort hold was resolved. Today's steward injects the old version and blocks a valid cutover. The team reopens records, reprompts agents and pulls an owner into a review that should not exist.
Measure the time to identify the stale version, reviewer minutes, missed release window and revalidation work. Also measure exceptions the steward actually catches. Without both sides, a dashboard rewards interruption volume.
At cutover, the host resolves the protected revision and sees that the owner cleared the cohort. It rejects the stale packet, records the correction and lets the service evaluate the current receipt. No guessing from memory.
Do that because a supervisor that creates more recovery work than it prevents is not productive. Set an intervention budget, compare with a simpler rule and retire the role if it cannot beat that baseline.
