In a fictional migration, the product brief says approved. Search returns it first. A separate backfill exception is filed under a cohort identifier. Both records matter, but only one reaches the worker.
The mistake is not that the brief is false. It authorizes a release window, not reconciliation for every affected cohort. Search ranking cannot grant authority to cut over.
Rewrite the query around decision prerequisites. Search the exception register, then have the host resolve the current cohort record. Better retrieval helps discovery; a protected source check covers missing hits.
Do that because the most relevant chunk can omit the one fact that changes the action. Grade whether the condition arrived before dispatch and whether the service refused cutover without its receipt.
