Picture a team after a customer rollout fails. The repository has ten AI-generated plans, twenty summaries and clean task boards. Every plan assumed a security approval covered all tenants. The original approval covered one.

The first task cites a summary, which cites a note, which cites a draft. None quotes the signed condition or names the person who could widen it. The team spends its response time reconstructing permission rather than fixing the product.

In a safe retrospective exercise, take one completed task and trace it to the exact decision passage. Mark where scope changed, who saw it and whether the final customer effect matched. Keep the receipt for future releases.

Do not ask every agent to write a longer summary. Require a source-linked condition and owner when a draft becomes an authorized task. Test whether the next agent can recover that condition before it acts.
