Fictional SaaS release: three agents update API, UI and migration code. CI is green. The one test for a migration-only tenant exception never ran because its fixture was missing. The worker summary says 'all tests pass.' Who has permission to say this is not verified?

A parser can report zero failures from an empty file. A dashboard can show green on yesterday's commit. Neither says the migration tenant received access while an ordinary tenant remained denied. The QA agent should identify the gap, and a trusted gate should refuse promotion until fresh evidence arrives.

In an isolated build, remove the entitlement guard and run the protected acceptance fixture against both cohorts. If the suite remains green, fix the oracle. If the fixture cannot run, mark unverified. Bind any eventual pass to the exact artifact and ask the release owner to inspect the receipt.

Give QA authority to stop for failed or absent required evidence. If an owner still chooses a bounded exception, record reason, cohort, compensating check, expiry and rollback. Do this because a warning that can be ignored without a receipt is not a release control.
